Tabia Health, Inc. (Tabia) today announced plans to combine American Heart Association CarePlans into Tabia’s AI-powered healthcare orchestration platform to improve CarePlan utilization and patient adherence in cardiac care. As part of this initiative, Tabia has joined the American Heart Association Center for Health Technology & Innovation’s (the Center) Innovators’ Network. The Center is focused on building and fostering health technology relationships to develop innovative and scalable solutions.

Tabia Health increases patient adherence with an innovative, AI-powered care orchestration platform that makes it easy and cost-effective for patients, providers and payers to stay on track and on time with scientifically proven care pathways, improving health and reducing long-term healthcare costs. Tabia is pioneering the digital availability combined with the clinical use of the American Heart Association’s CarePlans.

The American Heart Association has defined ideal cardiovascular health based on eight common risk factors, Life's Essential 8®, that people can improve through lifestyle changes: smoking status, physical activity, weight, diet, blood glucose, cholesterol, sleep and blood pressure. The Association’s CarePlans present actions people can take to improve their Life’s Essential 8 factors and are designed so anyone can work toward improved health.

The Innovators’ Network is a healthcare technology consortium that connects entrepreneurs, providers, researchers and payers. Innovators’ Network members also have access to the Association’s digital guidelines, recommendations and best-in-class science as they develop digital healthcare technologies. Members collaborate with the Center in different ways.
